Elfin Services is a family owned and operated business started in 1998 by Yvonne and Steve Polkow, after they spent several years helping older friends work through some difficult and complex personal issues. The Polkow’s realized there was great need for high quality advocacy for the elderly, and for experienced, compassionate Care Management and Caregiving services.

Elfin Services has grown steadily and carefully since its inception. Today the company employs several Care Managers, over 40 Caregivers, and has an ever growing number of clients. We provide a diverse range of services to the aging population. Elfin Services adheres to the highest ethical guidelines including those of the National Association of Professional Geriatric Care Managers, and the Society of Certified Senior Advisors, while also maintaining oversight, local rules and state laws to stay in compliance with the State Regulations and the Department of Health.

As a local company, Elfin eliminates the oversight of a franchised corporation allowing more flexibility and customization of care, which results in a more personal touch and connection with our clients and family members.


We establish a personal relationship with each client and use our understanding of their needs and wishes as the basis for our advocacy.  We offer:

  • 24/7 availability of Care Managers and Caregiving to assist with all personal needs in the home and Senior Living Communities including Assisted Living, Adult Family Home, Post-Rehab, etc.

  • Assistance with Activities of Daily Living (ADL's) including meal-prep, feeding, toileting, personal hygiene, light housekeeping, medication reminders, etc.

  • Nurse Delegated services including Diabetes Management; CBG monitoring, insulin injections, etc. Also includes tube feedings, wound care, skin care, etc.

  • Shopping and errands; including but not limited to groceries, clothing, medical supplies, prescriptions, etc.

  • Coordination of medical services; such as doctor’s appointments, OT, PT, etc., while also acting as your health care advocate and liason .

  • Transportation and assistance to medical appointments and medical emergencies, including after-hours checks and advocacy at the ER.

  • In-home evaluation of health, social, and safety needs.

  • Wellness Checks to ensure you or our loved ones are being properly cared for.

  • Problem solving, advocacy, and crisis management including supervised visits, assistance with government benefit applications and renewals, etc.

  • Coordination of alternative housing utilizing Senior Placement Specialists.

  • Transportation to personal, professional, and pet Veterinary appointments.

  • Coordination of community services and outings to stimulate social needs.

  • Power of Attorney for Healthcare.
